What to bring
Your insurance card, a photo ID, a list of any medications you take, and records from a past doctor if you have them handy.
Forms
We'll email you a short intake form when you book, so you can fill it out at home instead of in the waiting room.

Common questions
Do you see kids and adults?
Yes. We're a family practice, so we can care for the whole household, from babies to grandparents.
Can I get same-day care?
Usually, yes. Call before noon if you're sick and we'll do our best to get you in that day.
How do I get my results?
A nurse calls you, and everything is in your patient portal too. We don't leave you wondering.
